Nebraska Releases 2019 Nebraska Lottery Winner Claim Form

Nebraska has released a 2019 version of its Form NE-LOT, Nebraska Lottery Winner Claim Form, which is filed for the purpose of gambling winning withholdings.

The form only has one revision from last year’s version: winnings from the “Lucky for Life” game are now subject to withholding.
